You can create unique, custom forms that can be attached to each of your service, group or resource booking option.

  1. Go to Custom Form Management Page.

  2. In the Edit Custom Form modal, do these steps:

  • Edit or add a new form.

  • After creating or opening an existing form, select where the form should display.

  • In the service booking and group session widget tab, enable “Always show on booking widget” and select the Services you wish to attach to the custom form.

  • In the Contact Forms tab, enable “Always show on booking widget” and select the Services you wish to attach to the custom form.

3. Click Update.

Once done creating or updating custom form,

  1. Go to Booking Widget or Group Session Widget

  2. In Booking Widget Configuration, Select Form Field under Jump to: dropdown

  3. In the Booking Configuration: Step 6 / 9, enable custom forms

  4. Enable custom forms that was created that you wanted to show on the booking widget

  5. Click Save
